Your Questions, Answered.

Physical therapy can come with a lot of questions. Here are some answers to help you get started.

hip PT
  • No! Wisconsin allows direct access so you can schedule an appointment without a physician referral.

  • Elevate PT is a fee-for-service (cash-based) practice. This allows for longer one-on-one visits and care that's focused entirely on you, not insurance limitations. You can use FSA and HSA to cover services if you have them.

  • I specialize in sports and women’s health physical therapy, while welcoming patients of all ages and conditions. I have particular expertise in treating hip impingement/FAI, spinal conditions, and lower extremity injuries.

  • This is extremely fluid, but following your initial evaluation I will provide an estimate plan of care including number of visits. Additionally, I offer packages to reduce the price of return sessions (see below).

    • 10 treatments $1250 ($125/session)

    • 6 treatments $800 ($133/session)

    • 4 treatments $550 ($137/session)

  • Your initial evaluation will vary between 60-75 minutes typically. Follow up sessions are typically 45- 60 minutes all one-on-one with your physical therapist.

  • Wear comfortable clothing that allows movement. Athletic wear is best.

  • You can schedule online, email us at hello@elevatept.com or call/text 608-369-4329.
